Problem :

I am currently learning to use the windbg to debug the crash dump. I am facing following error. What is the meaning of “first/second chance not available” ?
This dump file has an exception of interest stored in it.
The stored exception information can be accessed via .ecxr.
(e38.2270): Access violation - code c0000005 (first/second chance not available)
How can I fix above error?

Solution :

I find your question very interesting so trying to answer it for you.

The(first/second chance not available) exception clearly suggests that the information about if the exception was of the first-chance or of the second-chance is missing from your crash dump file.

The first chance exception is amongst the exception which is handled by your debuggers first chance mechanism so that it can handle the exceptions.

A complete truth about none is available here clearly suggest that there were no exceptions thrown where your error must have occurred or it may suggest that there is no first chance handling mechanisms present in your debugger which can handle the relevant exception which was thrown.

I can surely say that above mentioned error clearly has no exceptions being thrown because of it and so it appears just the seg fault.

Problem: How to Check state of inputFile after opening and each inputFile >> numbers[count] operation separately to make sure there is no errors in loading from the file. Also consider what should happen if the file contained less than ARRAY_SIZE numbers. I would also suggest using std::array or std::vector which perform more checks in good debuggers.
